Client Panel

We invite you to join our Client Panel!

At SAG we strongly value the opinion of our patients. We are eager to learn from their experiences and feedback. That is why we have established the SAG Client Panel in 2018. Their input allows us to improve our services and quality of care.

Would you like to help us improve our services and health care (systems), by giving us feedback? Are you 18+ and registered at a SAG health center as a patient? Then we gladly invite you to participate as a member of SAG Client Panel!

What does a Client Panel member do?
As a Client Panel member you will receive a questionnaire by e-mail, twice a year (in both English and Dutch). We will ask about your experiences with – and your opinion on – topics relevant to one (or more) of our health centers, or to SAG as a whole. Or we will ask about topics, brought in by Client Panel members. Afterwards, we often discuss the results with a randomly selected number of members: what are the most striking conclusions and/or key learning points?

How to become a Client Panel member?
You can register using the form below. We will treat your personal and contact information as strictly confidential. We will use it solely for the purpose of sending the questionnaires. You can unsubscribe at any time by sending an email to: cliëntenpanel@sag-amsterdam.nl. In case you de-register as a patient: please don’t forget to unsubscribe the Client Panel as well.
