The practice assistant supports the general practitioner in the treatment of patients who have a chronic physical condition. In most cases this is diabetes, asthma, COPD (bronchitis and emphysema) or cardiovascular disease. The practice assistant checks whether you are properly adjusted to the prescribed medication. They guide you in the proper use of medication and signal when medication change is necessary.
You will receive information about your illness and instructions on how to carry out checks yourself. The practice assistants can also perform additional research, such as a lung function test or a heart film. The practice assistants also keep an eye on whether you receive sufficient help and they can give you information when extra help is needed. Finally, the practice assistants can offer help with smoking cessation.
