
Online patient portal
SAG offers an online portal for patients that are registrated with SAG. Through this online patient portal you can schedule an appointment or eConsult with your doctor, request a repeat prescription, ask your doctor questions, send in photos, view your medical file, etcetera.
You can do this 24/7 with the ‘MySAG-app’ (MijnSAG-app) on your mobile phone, or online at the patient portal-web page of your health care center.
Make an account
To use this portal, you need to have a portal-account (this service is for SAG-registered patients only). To make an account, go to the patient portal-page of your health care center (made available by Uwzorgonline.nl), see list below. Unfortunately, at this moment these pages are available in Dutch only; hopefully, translating options will be available soon.

You only have to register once as a new user (‘Make an account’ / ‘Maak een nieuwe account aan’). After registration you will receive a confirmation link by e-mail. You activate your account by clicking on the confirmation link within 24 hours of receiving the e-mail.
Note that a Dutch mobile phone number is required because of two step verification. Also, we need to be able to reach you in case of emergencies.
Download MySAG-app / MijnSAG-app
On the online portal page of your healthcare center, you can download the MijnSAG-app. You can set the MijnSAG-app to different languages: Dutch, English, Arabic or Turkish.

You can log in at the web page of the patient portal of your health care center (choose from the list above) or open the MijnSAG-app on your mobile phone (with two step verification).

Appointments
We only have consultation hours by appointment. We do not offer a walk-in consultation hour. Every doctor has his own patients. In principle, you can only make appointments with your own general practitioner. In case of an urgent matter and your own doctor is not present, you can go to one of the other doctors.

With the doctor’s assistant
At the counter of the health center, between 8 a.m. and 5 p.m..
For appointments with our healthcare providers we use an Text Message-service to remind you of the date and time. You will reveive the text message 48 hours in advance. For this we ask you for permission. Do you check whether we have your correct phone number?
Do you prefer not to receive a text message (anymore)? Let us know so we can disable this service for you.
Repeat prescription
You can request repeat prescriptions for medicines in different ways. Please note that not all prescriptions can be repeated without consultation with your doctor, for example sedatives/sleep medication, psychiatric medication and antibiotics.

eConsult
You can consult your doctor by e-consult. You can ask questions to your doctor through a secure internet connection. This service can be found in our online patient portal. For this you first must have a portal account.
For which questions is the e-consult suitable?
An e-consult is only suitable for questions that are not urgent. Examples include: lifestyle, medication, (results of) research, an earlier consultation, monitoring and follow-up in chronic disorders.
For urgent complaints, contact your health care center directly by phone. For complaints for which the general practitioner must (probably) physically examine you, an appointment needs to be made.
View medical file
You can view your medical file online in our patient portal. To use the portal, you first need to have a portal account (this service is for SAG-registered patients only). We offer an app version and a web version of the portal. Through both you can view your medical file.

Here you will find an overview of complaints and diseases for which you have visited your doctor. - Medicines
Here you can see the overview of your medicines, as known to us in practice. - Hypersensitivities
Are you allergic or intolerant to something? Then you will find an overview of those substances here. - Results of research
View the results of laboratory tests here, for example a blood test. - Measurements
For example, did we measure your blood pressure or weight? You can find this here. - Letters
Letters to and from other healthcare providers, such as a referral letter, can be viewed here.
MySAG-app / MijnSAG-app
To view your medical file via the MijnSAG-app, you can download the app on the patient portal-page of your health care center; look for the right (web) portal page in the list at the top of this page.

Do you have any questions about the content of your file? Then you can send us a secure message via the patient portal (MijnSAG-app or web page).
Protected data
Your data is protected
The patient portal is hosted in a secure environment. That means that your data is protected. All data is transmitted encrypted and stored encrypted on secure servers. Only you and your healthcare provider have access to your data. Every time you log in, you will receive a verification code by text message (two step verification). This is also to guarantee your safety.
Access to the patient portal
To be able to use the patient portal, we ask you to register once as a new user (make an account). After registration you will receive a confirmation link by e-mail. You activate your account by clicking on the confirmation link within 24 hours of receiving the e-mail.
To protect your privacy, we ask you to come to your health center and show your ID. In that way make sure that you are the one who gets access to your account in the portal. After identification you can use the online services.
Permission LSP
Electronic exchange of your medical data: Have you already given permission?
If you visit a doctor or pharmacy, you expect to be helped properly. Also in the evening, at night and at the weekend at the doctor’s office or the service pharmacy. But over there the caregivers do not know you, nor your health situation. That is why they need up-to-date information about your health. Electronic exchange of your medical data takes care of this. This way you always receive the right care. But that is only possible with your permission.

You arrange your permission verbally or in writing with your doctor and pharmacist. Please visit www.ikgeeftoestemming.nl to find out how you van arrange your permission. To arrange online permission you need DigiD with text message function.
Landelijk SchakelPunt (LSP)
Most patients are being treated by more than one health care giver. For instance: for treatment a patient visits a GP, a hospital, a specialist or several specialists, etcetera. In order for these health care givers to be able to look up information on the other health care givers that are treating you (so they can see who is treating you, why they are treating you and how to contact them), you need to give permission to the Landelijk SchakelPunt (LSP). The LSP doesn’t store your medical data. The LSP only provides the electronical exchange of data amongst health care givers.
