Older people and chronically ill people often take a lot of medication for a long time. That involves risks.

The chance of unintended interaction between the various medications or unintended side effects increases when the number of drugs gets larger. It is also difficult for many patients to make no mistakes in their use of medication and to continue using the drugs as they are intended. With more than six medicines, the patient’s safety is really at stake.

To ensure that the quality and safety of medication use in this target group is guaranteed, the SAG health centers have started a polypharmacy program. Together with patients, general practitioners and pharmacists look for the perfect medical treatment to ensure the safety of patients.
