Elderly over the age of 75 have an increased risk of serious health problems. This is why the SAG has set up a special project for Vulnerable Older People. This project aims to map out all patients aged 75 and older and provide care where necessary.

All patients who are over 75 years who are unsufficient in the sights of the general practitioner are invited by a practice assistant or visited at home. On the basis of a general questionnaire, the practice supporter looks at whether further research is necessary. If necessary, a follow-up plan is drawn up in consultation with the patient and the general practitioner.
